Submission Checklist
In 250 words or less, tell us how your DFF, used both compassion and creativity to help you (or a loved one) understand, manage and live with diabetes.
  • Nominees will be judged on their compassion (70%), and creativity (30%) in helping to educate and motivate the person with diabetes to better manage their diabetes. Specific examples of their efforts must be cited.
  • Entrants may nominate more than one DFF, but each nomination must be a separate entry.
Videos are not required for judging purposes and will not increase the chances of winning.
Six DFFs and Their Nominators Each Will Win
To submit a nomination video you must have a YouTube account or you can create a new account.
Then follow these instructions:
  1. Make sure you check the proper box in step 3 before clicking the "Post Your DFF Story" button.
  2. On the following confirmation page click the "Submit Nomination Video" button.
  3. You will be taken to YouTube and must click "Allow Access" to allow your video to be uploaded. You may need to log in to YouTube first.
  4. Click the "Choose File" button, select your video.
  5. Click "Upload Video File" and remain on the page until told the upload has finished.
Video entries must be at least thirty (30) seconds in length, but no more than two (2) minutes.

Prizes
  • One (1) Grand Prize: $10,000, ($5,000 for the DFF and $5,000 for the nominator), and a case of Dreamfields Angel Hair pasta ($51.80) each. The Approximate Retail Value (ARV) is $10,131.80.
  • Four (4) First Prizes: $2,000 ($1,000 for the DFF and $1,000 for the nominator) and a case of Dreamfields Angel Hair pasta ($51.80) each. The Approximate Retail Value (ARV) is $2,131.80.
  • One (1) "People's Choice" Prize: $2,000 ($1,000 for the DFF and $1,000 for the nominator) and a case of Dreamfields Angel Hair pasta ($51.80) each. The Approximate Retail Value (ARV) is $2,131.80.
  • Thirteen (13) DFF of the Week Winners: a case of Dreamfields Angel Hair pasta ($51.80) to both the DFF and the nominator. The Approximate Retail Value (ARV) is $131.80.
  • Each entrant: one coupon for a $1-off a box of Dreamfields pasta. Prizes may not be transferred, sold or bartered.
Judging Criteria Final judging will be conducted on or before Friday, May 28, 2010, by a panel of representatives selected by Sponsor. Entries will be judged by the following criteria:
  • Compassion (70%): How did the DFF work with the individual to meet his/her unique diabetic needs?
  • Creativity (30%): What unique or innovative methods did the DFF use to support the person with diabetes and help him/her better manage his/her diabetes.?
Before the final judging, finalists (both entrants and nominees) will be asked to sign an affidavit regarding adherence to the contest rules. Entries that contain any indecent or obscene materials, sexual content, inappropriate language, unsuitable behavior or adult material will be automatically discarded and will not be eligible for further consideration. All decisions of the judging panel will be at their sole discretion and will be final in all matters relating to this contest. Entry All entries must be original essays created by the entrant to nominate a special person who has done an extraordinary job in helping someone understand, manage and live with diabetes.

In other words, just describe how this person was a DFF - a Diabetes Friend Forever. Timing The DFF Contest begins on January 29, 2010, and ends on April 30, 2010, at 11:59 p.m. EST.
Eligibility DREAMFIELDS® Diabetes Friends Forever (DFF) Essay Contest (Contest) is open to legal residents of the fifty (50) United States and the District of Columbia who are at least eighteen (18) years old at the time of entry. Employees of DNA Dreamfields Company, LLC (Sponsor), Dakota Growers Pasta Company, Inc., M&P Food Communications, Inc., HyperDrive Interactive Ltd., The Zimmerman Group, and any of their parent and affiliate companies as well as the immediate family (spouse, parents, siblings and children) and household members of each such employee are not eligible. The Contest is subject to all applicable federal, state and local laws and regulations. Void where prohibited. No purchase necessary to enter or win.
How To Enter
  • Visit www.diabetesfriendsforever.com and follow the links and instructions to complete the nomination for the DREAMFIELDS® Diabetes Friends Forever (DFF) Essay Contest.
  • Entries can also be mailed to DREAMFIELDS® Diabetes Friends Forever (DFF) Essay Contest PO Box 1896, Chicago, IL 60690. Entries sent via the U.S. Postal Service must be postmarked by midnight April 30, 2010, and received no later than May 5, 2010.
  • Entries must describe how someone has been a DFF by using compassion and creativity to help you (or a loved one) understand, manage and live with diabetes. Specific examples of their efforts must be cited in your story.
  • Each entry must include a valid e-mail address of both the entrant and nominee. You may submit your telephone number if you are unsure about the nominated DFF's e-mail information.
  • Submission of your e-mail address grants the Sponsor permission to send you e-mail(s) which will include a link to Dreamfields Pasta online coupon, that can be accessed online via SmartSource online coupon technology. You may unsubscribe to Dreamfields e-mail anytime via a link at the bottom of all e-mails we send to you.
  • Entries must not exceed two hundred fifty (250) words in length.
  • Entrants may nominate more than one DFF, but each nomination must be a separate entry.
  • A photo entry may also accompany a written entry, but is not required for judging purposed and will not increase the chances of winning.
  • A video entry may also accompany a written entry, but is not required for judging purposes and will not increase the chances of winning. Video entries must be at least thirty (30) seconds long, but no longer than two (2) minutes. Videos must be submitted in one of the following file formats: .MPEG; .MOV; .AVI; .WMV; .FLV and are processed through YouTube.com. All entries must be submitted in English and describe how someone has been a DFF by using compassion and creativity to help you (or a loved one) better understand, manage and live with diabetes. Specific examples of their efforts must be cited in your story.
  • All entries become the exclusive property of Sponsor, and none will be returned. By entering the Contest, entrant irrevocably grants to Sponsor and its agents the unconditional and perpetual right to post, display, edit, adapt and publish any or all of the nominations submitted in any media (including online) without attribution or compensation to the entrant, nominee, his or her successors or assigns, or any other entity unless prohibited by law.
  • Submission of an entry in the Contest constitutes entrant's irrevocable assignment, conveyance and transference to Sponsor of all right, title, and interest in the entry, including, without limitation, all copyrights.
  • Entrants must comply with the following requirements and represent and warrant that the nomination submitted: is the entrant's original creation; does not infringe any other person's or entity's rights including but not limited to intellectual property rights; has not been published in any medium or submitted in any other contest of any kind.
  • Sponsor, in its sole discretion, may disqualify an entry to the Contest if it believes that the nomination fails to conform to the guidelines outlined above.
  • Sponsor assumes no responsibility for lost, late, misdirected or damaged entries, including entries that may be delayed in transmission. No incomplete, illegible or otherwise invalid entries will be accepted.
  • Sponsor reserves the right, in its sole discretion, to change these Official Rules, adjust the dates herein or cancel the Contest.
